How can I update my beneficiary?
You can update your beneficiary(ies) in one of the following ways:
- Online
Log in to your service provider’s website and update your beneficiary information directly within your account. - By phone
Contact your service provider using their Client Service Phone Number. A representative can assist you with updating or changing your beneficiary(ies) over the phone. - Paper form
Complete a beneficiary designation form and submit it to your Plan Administrator, following the instructions provided.
When adding or updating a beneficiary, you will typically need the following information for each individual:
- Full legal name
- Date of birth
- Social Security number
- Current address
